This permit will be submitted to Modesto Police Department by the MJC Foundation Office.  An MJC representative must fill out this form.  If you are requesting alcohol for your event, see the instructions below for the sections of this permit that you will fill out and then send to Sandy Marks. Please do not submit to MPD.

Applicant Information Section
  • NAME OF PERSON IN CHARGE OF EVENT
    • This is the person who is the contact point for the event and will be present at the event for its entire duration. 
  • ADDRESS
    • This can be the college address or the “person in charge’s” home address. 
  • HM PH/BUS PH 
    • This number should be one that will be available during the event. 
  • DRIVERS LICENSE #
    • The “person in charge” must provide their driver’s license information. 
  • SIGNATURE
    • This is the signature of the “person in charge.” 

Event Information Section
  • LOCATION OF PERMITTED ACTIVITY
    • Where you are holding your event (alcohol permits do not supersede space reservations with Events-Facilities).
  • DATE
    • The scheduled (and verified with Events-Facilities) date of your event.
  • DAY
    • What day of the week.  START TIME: Time that the event starts (not including set-up time) am/pm     END TIME: Time the event ends (not including clean-up or tear down) am/pm 
  • LIVE ENTERTAINMENT
    • There are heck boxes for yes or no to live entertainment. 
  • TYPE
    • (Band/Singer/DJ, etc.) : Indicate the type of entertainment you will be providing.   
  • NAME OF ENTERTAINMENT
    • The name of the band, DJ, etc. 
  • EXPECTED ATTENDANCE
    • Indicate the number of anticipated guests (this question is critical as it determines the required security for an event. The ration is one officer for every 100 guests).
  • AGE GROUP EXPECTED
    • The age ranges of your anticipated guests (this is also critical as if you are having anyone under 21, you will need to provide some type of ID checking and identification for those ID’d over 21 i.e. wristbands, etc.)
  • TYPE OF DRINKING CONTAINERS
    • Identify what you are using to serve the alcohol, i.e. plastic cup, wine glasses, etc.
    • ACTUAL LOCATION OF BEVERAGE SERVICE ON PROPERTY: Where will you serve the alcohol, i.e. Behind a bar, at the tables, etc.   
  • TYPE OF BEVERAGE BEING SERVED
    • (wine/beer/etc.) What alcohol will you have (YCCD alcohol policy only allows for beer or wine to be served at an on-campus event).
