Some students are required to submit copies of Tax Schedule 1, 2, and/or 3 that were filed with their tax return. Please confirm on your Financial Aid Document Checklist if you need to submit this document.

Don’t have a copy? Please request your Tax Return Transcript from IRS and submit to our office.

  • Available from the IRS by calling 1-800-908-9946
  • Follow prompts to enter your Social Security number and the numbers in your address
  • Select Option 2 to request an IRS Tax Return Transcript and then enter appropriate year
  • Go to the IRS website
  • Choose to "Get Transcript ONLINE" (immediately), OR "Get Transcript by MAIL" (5 to 10 days)
  • Follow steps on the IRS website to complete the request
  • Select Return Transcript for the appropriate year
  • Form 4506-T, to access form click on 4506-T
  • You can MAIL or FAX form. Instructions on 2nd page of 4506-T form
